I/We the undersigned, hereby make an application for membership to the Camp Hilbert Swim Club of the Carole & Marcus Weinstein Jewish Community Center. I/We agree to abide by its rules and by-laws. Membership is neither transferrable nor nonrefundable and will run from May 28, 2023, through September 4, 2023. Membership at the Camp Hilbert Swim Club is a privilege that may be revoked with or without cause at any time by the Facility Director, CEO, or the Board of Directors.

Participation in any Camp Hilbert Swim Club activities and use of the recreational facilities involves risk of accidental injury despite all safety precautions. I/We will assume all risks(injury or illness) to our children or me/us that may occur during participation in any activities or use of facilities at the Camp Hilbert Swim Club of the Weinstein JCC. I/We understand that an adult member must accompany children under 13 years of age.

By Signing this form, I/We acknowledge that I/We are aware of the potential risks of participating in activities and/or programs at the Camp Hilbert Swim Club and agree, to in no way, hold management, agents, or employees of the Weinstein JCC liable for any injury I/We or members of my/our family sustain.
